Spironolactone for Female Acne

A patient advice sheet — 8 practical points. Reviewed and updated by Dr. Abdulaziz AlRashed, July 2026.

The advice

  1. Spironolactone is an effective, safe long-term option for hormonal-pattern acne in adult women (the type flaring on the jawline and around the period).
  2. Results take 2–3 months to show, and it is usually combined with topical treatment — patience is essential.
  3. Possible mild, dose-related effects: irregular periods, breast tenderness, more urination, or slight dizziness (drink enough water).
  4. No pregnancy while using it: use effective contraception — it is often paired with the contraceptive pill, which itself helps acne.
  5. Potassium blood tests are usually unnecessary in healthy young women; we may check depending on your case or other medicines.
  6. Avoid potassium supplements and heavy excess of potassium-rich sources, and tell any doctor you take it (especially with blood-pressure medicines).
  7. Tell us immediately when planning pregnancy so it is stopped and the plan adjusted.
  8. It is not used for acne in men (hormonal side effects).
